Description
Background- Broad ligament hematoma is an unusual complication (1:500 and 1:20,000) that can occur during delivery, just after delivery as pain is not of a conspicuous nature and so also the vaginal bleeding.
Case -A 23 year G2P1L1 post vaginal delivery (compound presentation) with acute hemodynamic instability. A high level of suspicion, blood products and the use of imaging modalities needed Exploratory laparotomy done - rent in lower segment with intact serosa with left broad ligament hematoma present . Bilateral internal iliac ligation with hysterectomy done.
Conclusion- Though uncommon, broad ligament hematomas should be considered in postpartum women presenting with anemia and vaginal bleeding, even without hemodynamic instability.
