Abstract
Introduction- Bilobate placenta is an abnormality of placenta having two roughly equal sized lobes separated by a membrane. It occurs in 2-8% of placentas. It is very rare entity, can be missed during ultrasonography and during caesarian section may lead to dreaded fetal and maternal complication Vasa praevia may be associated with bilobate placentas. Bilobed placentas are associated with increasing maternal age and with a maternal history of infertility, cigarette smoking, diabetes mellitus.
Case report- 24-year-old female admitted with Antepartum hemorrhage with placenta lying in upper segment of uterus in ultrasound report. On admission her general examination findings and vitals were stable. During c-section 1 lobe of placenta found in upper segment and 2nd lobe found in lower uterine segment which was completely covering the os.
Conclusion - Bilobate placenta is a morphological abnormality, the ultra sonography guided recognition of which in the antenatal period is very important. It could be missed by ultrasonogrphy so its clinical examination must be carried out very carefully for the management of the patient during delivery. The vessels connecting the main placenta with the bilobate lobe may rupture during labor and lead to fetal demise, Later on it may lead to uterine sepsis. Accompanying vasa praevia might cause dangerous fetal hemorrhage at delivery. This case is atypical presentation of placental abnormality which was undiagnosed in ultrasonography evaluation while identified during clinical examination which establishes the significance of clinical judgment.
